General Terms and Conditions

1. Scope

These general terms and conditions apply to the contractual relationships between the Dentamar dental clinic in Bağdat Cad. No:340 Daire:6 34728 Erenköy/Kadıköy/İstanbul, Turkey and the patient who uses the dental care services. The T&Cs apply to all dental, dental surgical and dental technical contractual relationships, to the fulfillment of obligations and to the assertion of claims from specific individual services received.

If the treatment contract is concluded with a minor or a person with limited legal capacity, it requires the signature of the legal representative.

2. Definitions

a. Healthcare provider

Dentamar dental clinic in Bağdat Cad. No:340 Daire:6 34728 Erenköy/Kadıköy/İstanbul, Turkey. (Hereinafter: service provider).

Dr. Yilmaz, the sole authorized representative, operates the

Dentamar dental clinic in Bağdat Cad. No:340 Daire:6 34728 Erenköy/Kadıköy/İstanbul, Turkey. The health services are provided through the activities of its members and employees.

The representative of Dentamar dental clinic in Bağdat Cad. No:340 Daire:6 34728 Erenköy/Kadıköy/İstanbul, Turkey. declares that he is in possession of the professional, administrative and operating licenses, conditions required to provide the services in Turkey.

b. Service recipient

The patient of the Dentamar dental clinic in Bağdat Cad.No:340 Daire:6 34728 Erenköy/Kadıköy/İstanbul, Turkey, who is named in the individual service contract, in the treatment and cost plan of Dentamar in Bağdat Cad.No:340 Daire:6 34728 Erenköy/Kadıköy/İstanbul, Turkey or in another declaration of consent to provide a service and who uses the health service (hereinafter: service recipient).

3. Health service

The service provider provides its dental services in accordance with applicable Turkish law.

The service provider declares that it meets all personnel and material requirements necessary to provide its services and that it is in possession of the necessary official permits for its medical activity.

When using the services, the contracting parties are obliged to cooperate. As part of this duty to cooperate, the service recipient must inform the service provider or the treating dentist about all circumstances and facts (in particular previous illnesses, previous treatments, medication and medicinal products, health-damaging risk factors, etc.) that are necessary to create a diagnosis or a corresponding treatment plan and to carry out the planned interventions. The service recipient's duty to provide information also extends to all circumstances (in particular infectious diseases and those illnesses that are associated with occupational disability, etc.) that endanger the life or physical integrity of others.

The service recipient is obliged to appear at the dental clinic in an appropriate physical and psychological state to use the dental service; otherwise the service provider has the right to refuse the service.

4. Payment arrangements

Before using the service, the service recipient must pay a deposit to the agency currently cooperating with the service provider, in the name and on behalf of the service provider. A deposit of 20% is due upon delivery of the travel confirmation.

The final payment of the amount remaining after deduction of the deposit is made by the service recipient on site at the service provider's clinic (in cash or credit card).

For bookings made less than 7 working days before the start of the trip, the full amount is due upon arrival on site.

5. Passport, customs and health regulations

The service recipient is responsible for complying with the applicable passport, visa, customs and health regulations in Turkey. In particular, the current announcements from the Federal Foreign Office must be observed.

6. Use of data

If you are a service recipient with us or have purchased one of our products or used our services, we will use your telephone number and email address to send you information about advice and advertising via messenger, email or phone call. You can object to the use for this purpose at any time, by telephone, email, messenger or post. We use your data exclusively in connection with our services and products.
